UPDATE 5/20/2021:

The clinic is now open for some patient appointments, medication pickups, and screening paperwork. Please do not enter if you have current symptoms of COVID-19 or have recently been exposed to COVID-19.

MOM has reopened for a limited number of in-person patient visits. Many appointments will still be conducted via telehealth. Patients will be contacted via email, text, and/or phone prior to their appointments to confirm whether it is in-person or via telehealth. Patients should not come to the clinic for an appointment unless instructed to do so.

MOM remains OPEN for medication refills. Please call in your refill request to (334) 281-8008 and use the automated system or use the Medication Refill Request online form. You will receive a text or phone call when your medication is ready for pickup.

PLEASE NOTE: MOM is not providing COVID-19 testing.

  1. Patient appointments will now be conducted by a combination of telehealth and in-person visits. The clinic will call or text you prior to your appointment to go over the procedures with you. For telehealth appointments, please be ready to answer your provider’s call during the time frame that the clinic gives you. Please do not come to an in-person visit if you have any COVID-19 symptoms. Call the clinic for instructions.

  2. The No-Show policy will be reinstated for all in-person and telehealth appointments. Please call to reschedule if needed to avoid being charged as a no-show. No-shows will not count for any patient who calls with a new development of COVID-19 symptoms.

  3. If you have a new development of any of these symptoms - shortness of breath, cough, and/or fever - call (334) 281-8008 to speak to someone about whether you should be tested for COVID-19.

  4. Medication refills MUST be called in to (334) 281-8008 or use the Medication Refill Request online form. No requests will be taken in person. Patients should only come to pickup medications if the patient is symptom-free and only when they have received a call that medications are ready.

  5. Financial Screening for New and Current Patients will be conducted over the phone and/or through the mail. Click here for an application to print. Call (334) 281-8008 for help filling out your application.

  6. MOM will be instituting additional cleaning measures to maintain the cleanliness of the clinic for the safety of our patients, volunteers, and staff.

  7. Keep yourself, your family, and the clinic virus-free by practicing good hygiene - wash your hands regularly and cover your cough with your elbow.

  8. PRAY for God's protection over the clinic and all who receive care and serve here.
